Atracurium for Injection

Description – Atramon is available in liquid injection

Indications/uses – Endotracheal Intubation

                               Mechanical Ventilation

                               Skeletal Muscle relaxation during surgery

Dosage – An initial Atracurium Besylate dose of 0.3mg to 0.6mg per kg (depending on the duration of full blocked required), given as an intravenous bolus injection is recommended. This will provide adequate relaxation for about 15 to 30 min.

Endotracheal intubation can usally be accomplished with in 90 to 120 seconds of the intravenous injection of 0.5mg to 0.6mg per kg, maximum neuromuscular blockade is generally achieved approximately 3 to 5 minutes after administration

Contraindications – Atracurium is contraindicated in patients known to have a hypersensitivity to it and in neonates. It advantage is that, it does not require any dosage adjustment in renal or hepatic disease as it spontaneously degrades in the plasma.

Special Precautions:    Dehydration or Electrolyte Abnormalities

                                     Situation in which Histamine release would be problematic

                                     Fracture or Muscle spasm

                                     Muscle spasm

                                     Hyperthermia

Adverse Drug Reactions: 

  • Bronchospasm
  • Hypotension, Tachycardia
  • Flushing, rash
  • Allergic reactions including ANAPHYLAXIS

Drug Interaction:

  • Intensity and duration of paralysis may be prolonged by pretreatment with Succinylcholine, general anesthesia (inhalation), aminoglycosides (vancomycin), tetracyclines, Polymyxin b, Colistin, Clindamycin, etc
  • Higher infusion rates may be required and duration of action may be shortened in patients receiving long term Carbamazepine or Phnytoin

Mechanism of Action: it is a non-depolarizing agent antagonize the neurotransmitter action of acetylcholine by binding competitively with cholinergic receptor sites on the motor end plate, this antagonism is inhibited and neuromuscular block reversed by acetylcholinesterase inhibitors such as neostigmine, edrophonium and pyridostigmine
